What is Azoospermia?
Azoospermia is a medical condition in which there is no sperm in a man’s semen. Azoospermia makes natural conception nearly impossible, and is usually referred to as zero sperm count. Azoospermia is most prevalent in approximately 10–15% of all infertile men and approximately 1% of the male population. Azoospermia can be a frustrating condition, but can actually be effectively treated and managed with the correct diagnosis.
A man with azoospermia has no sperm in his semen after a lab evaluates it. This absence of sperm may be due to either a complete lack of sperm production or a blockage preventing secretion of sperm.
Azoospermia and a low sperm count (oligospermia) are separate conditions. In the case of oligospermia, sperm are indeed present but in measurably insufficient amounts, while in azoospermia, no sperm are present in the ejaculate. This also affects the treatment of either condition.
Though azoospermia may be somewhat rare when compared to the rest of male infertility, it is still a significant factor in male infertility. Many of the advanced techniques in reproductive medicine today, such as TESA, Micro-TESE, and ICSI, can potentially help azoospermic men become biological fathers, but depend on the individual’s diagnosis and overall fertility condition.

Yes, many men with azoospermia can still become biological fathers, depending on the underlying cause of the condition. Advances in reproductive medicine have made it possible to retrieve sperm directly from the testicles or epididymis, even when no sperm are present in the semen.
For men with obstructive azoospermia and selected cases of non-obstructive azoospermia, sperm retrieval techniques such as TESA, PESA, or Micro-TESE can be used to collect viable sperm. The retrieved sperm are then used in IVF with ICSI (Intracytoplasmic Sperm Injection), where a single healthy sperm is injected directly into a mature egg to achieve fertilization.
In situations where sperm cannot be retrieved despite advanced treatment, donor sperm may be discussed as an alternative option. However, this is typically considered only after a thorough evaluation and when other appropriate treatment options have been explored.
